This form is used to cancel all financial aid awards for the indicated term(s).
This form may be used by a student who:
will not attend at least one semester at OTC during the 2014-2015 award year.
is not planning to enroll at OTC during the award year.
will be receiving financial aid at another institution during a term in the award year.
does not wish to receive financial aid at OTC, including federal, state, or insititutional aid, during the award year for any reason.
I verify I am cancelling all financial aid awards at OTC for the term(s) indicated.
I confirm and understand all financial aid awards for the term(s) indicated will be cancelled.
