Any course additions, course drops, and withdrawals must be reported to the OTC Certifying Official. Failure to do so may result in overpayment by the VA. The VA will hold you responsibile for any overpayment of your educational benefits.

The submission of this form is for notification only. The submitting of this form does not drop, add, or withdraw you from class/es it just notifies the OTC Certifying Official to adjust your certification for the semester. You are still RESPONSIBLE to drop, add, or withdraw by the appropriate processes.
