Identify & Statement of Educational Purpose(to be signed at St. Thomas University)



(1)The student must appear in person at St. Thomas University  Bobcat Enrollment Center  to verify his or her identity by presenting a valid government-issued photo identification (ID), such as, but not limited to, a driver’s license, other state-issued ID, or passport. The institution will maintain a copy of the student’s photo ID that is annotated with the date it was received and reviewed and the name of the college official authorized to collect the student’s ID. 
 
This form must contain an original signature. Faxes and copies will not be accepted.

In addition to the ID document, the student must sign, in the presence of a St. Thomas University official, the following Statement of Educational Purpose. If the student is unable to appear in person at St. Thomas University to verify his or her identity, the student may sign the Statement of Educational Purpose in front of a notary public and must provide the following to St. Thomas University Bobcat Enrollment Center: 
 
  •  A copy of the valid government-issued photo ID (cannot be expired) that is acknowledged in the notary statement below, such as, but not limited to a driver’s license, other state issued ID or passport; and
  • The original notarized Statement of Educational Purpose provided below. We need a “wet” signature which means we cannot accept a faxed or scanned copy of the signed Statement.

If the student is unable to appear in person at St. Thomas University to verify his or her identity, the student must provide:

(3) If you have not yet provided your official high school or high school equivalent credentials to St. Thomas University as part of your admission process, you must submit a copy of your high school diploma, final high school transcript, General Educational Development (GED) certificate, home school transcript signed by parent or guardian, secondary school completion credentials for home school, or state certificate for completion of a state recognized high school equivalency test. Alternatively, you can submit an academic transcript showing successful completion of at least a two-year program that is acceptable for full credit toward a bachelor's degree.

Include on the copy of the valid government - issued identification the following information:

  • Original "Document Presented" stamp (if available), or statement that the original document was presented.

  • Full name of the representative accepting the documentation.

  • Date received.


By signing this form each party will certify that all information provided on this form is complete and correct, to the best of your knowledge. If you purposely give false or misleading information on this worksheet, you may be fined, be sentenced to jail or both.

Please note that the Statement of Educational Purpose must have “wet” signatures. This means that this form CANNOT be faxed or scanned to us.
