UWF Sponsored Event Accommodation Request Form

General Information

For disability accommodation requests only. Please do not use this form to RSVP for an event.

The University of West Florida is committed to providing a welcoming, accessible, and supportive environment for people with disabilities. To request a disability accommodation for a UWF sponsored event, submit this form at least two (2) business days prior to the scheduled event. If an event is posted less than two business days prior to the event, please submit this form as soon as possible. The university will make reasonable efforts to provide the accommodations.

Please note that the university does not provide wheelchairs, scooters, or other mobility devices. Guests and visitors must bring their own mobility devices.

Accessible parking spaces are available for vehicles with official disability plates or placards. Vehicles parked in accessible parking spaces without official disability plates or placards may be ticketed/towed.
