Dietetics intent applicant will complete the Prerequisite Form once, with all courses and final grades reported, no later than January 4, 2021. The Prerequisite Form requires the applicant to list the course number, title, university, the year it was taken, credit hours, and the final grade for each completed prerequisite course. Dietetics intent applicant will provide all transcripts to the Office of Records and Registration (emich.edu/registrar) no later than January 8, 2021.
