UAM Alumni Awards Nomination Form

If you expect you need more time to complete your submission, we recommend exiting this form and composing your recommendation in a word processor and then coming back to this form and use copy and paste to complete your recommendation.

The following items must be submitted in order for the nomination packet to be considered complete:

Completed nomination form
A copy of the nominee’s resume, CV, Linkedin profile, or similar professional summary
One letter of support (from individual other than the nominator) who can speak to the nominee’s accomplishments.
Optional: published articles featuring the nominee or other items of interest

NOMINATION DEADLINES:

Achievement and Merit Award – August 15^th
Continuing the Connection Award – August 15^th
Young Alumni Rising Star Award – August 15^th
Distinguished Alumnus/Alumna Award – February 15^th

Category of Nomination (select category):

Achievement and Merit Award – All living graduates and former students of UAM or Arkansas A&M. The award honors those alumni whose achievements in their vocational fields have brought distinction to themselves, credit to the University, and real benefits to their communities and profession.

Continuing the Connection Award –All living graduates and former students who attended Arkansas A&M. The award honors those alumni who best keep alive the connection between Arkansas A&M and UAM.

Young Alumni Rising Star Award – All living graduates and former students of UAM. The nominee must be under the age of 36 (35 years old or younger). The award honors our young alumni whose achievements in their vocational fields have brought distinction to themselves, credit to the University, and real benefits to their communities and their profession.

Distinguished Alumnus/Alumna Award – The award honors those alumni who have made significant contributions to society, and whose accomplishments, affiliations and careers have honored the legacy of excellence at the University. Nominees will be accomplished in three distinct areas as outlined below.

Professional Achievement: The recipient will have demonstrated achievements in their vocational field, having brought distinction to themselves and formed a record of accomplishments.
Service to Community: The recipient exhibits integrity and a demonstrated commitment to service and bettering the lives of others through volunteer efforts.
Service and Support to UAM: The recipient’s deeds and actions reflect the importance of his or her educational training, pride in alma mater and loyalty to UAM, as demonstrated through their interest in and support of the University.

Submit the completed nomination packet on or before the stated nomination deadline. Be sure to attach a copy of the nominee’s resume, CV, Linkedin profile, or similar professional summary and attach one letter of support (from individual other than the nominator) who can speak to the nominee’s accomplishments.
