The information you provide throughout this questionnaire will be used to
review your economic eligibility for the
Arthur O. Eve Educational and Higher Education Opportunity Programs at Cornell University. Please provide as much information as possible. All information you provide will be reviewed by a staff member in the Office of Academic Discovery and Impact (OADI) to determine your eligibility for the programs. Additional documentation will be required as proof of income and benefits. Materials should be submitted through the Collegeboard IDOC process as outlined by
Cornell University Office of Financial Aid and Student Employment for prospective students.
Please note, there is a
45 minute timer on each page, not the entire form. The timer will count down in the upper corner to make sure you are able to complete the required information.
Rememer to have your family income and expense information available. If you have been able to complete your
TAP application, please attach a copy of your
confirmation email to the end of the form.
If you have not been able to complete the TAP application, you will need to submit a copy of your confirmation email once it is complete.
Please contact the office and we will be glad to answer any questions. It is imperative you provide the information requested below as soon as possible. Any delay in receiving requested materials could hinder the chances of your application being processed for EOP/HEOP eligibility.
