Participant Commitment Agreement
To be a participant in good standing with TRiO, I understand that my responsibilities are as follow:
I. Every semester my TRiO coach and I will assess my academic standing and develop an academic plan, 2) review my academic plan and conduct a mid-semester progress review, and 3) discuss my academic goals and plan next semester's schedule.
II. I agree to meet with my TRiO Coach at least twice each semester.
III. I agree to attend each class everyday while it is in session unless otherwise restricted.
IV. I agree to attend tutoring when needed until my grades are improved or until the assignment has been completed.
V. I agree to attend a minimum of one TRiO sponsored workshop or activity each semester.
VI. I agree to speak with a financial aid counselor at least once each semester.
VII. I agree to contact my TRiO advisor anytime a change is made to my class schedule.
VIII. I agree to attend one office hour for each of my classes each semester.
IX. I agree to notify my instructors of any absences and make up any missed assignments as soon as possible.
X. I agree to follow the OCC code of conduct.
XI. I agree to abide by OCC's COVID-19 protocols.
XII. I agree to notify TRiO staff of changes in my address or phone number, to respond in a timely manner to phone calls, e-mails, and letters from the office.
XIII. I agree to allow my name and/or picture to be published in the program newsletters, publications, displays, and/or websites if I make the Dean's List, graduate, earn a scholarship or award, or participate in a TRiO event.
XIV. If I miss more than 2 appointments (counseling, mentoring, or tutoring) without calling to cancel, I understand I may be released from the program.
Additional Requirements for Students on Academic Probation
If you are on academic probation, this would apply:

Program Dismissal - if the program requirements are not met, you will be dismissed from the program. You will have to appeal for reinstatement.
I understand that I must sign the Participant Commitment Contract to be officially accepted into the TRiO program. I agree to the terms of this contract and if I have any changes in my contact information, I will contact my TRiO advisor.
