Unusual Enrollment History

Moraine Park Technical College recently received your Free Application for Federal Student Aid (FAFSA) information for the 2025-2026 school year. Based on review of your financial aid records, the Department of Education has determined that you have an Unusual Enrollment History (UEH). A student with this determination on their FAFSA record must provide additional documentation to the school before they are determined eligible to receive further financial aid.

**Processing of your financial aid has stopped until we receive this and all other requested documentation**

1. Please provide official transcripts* from each institution you attended and from which you received financial aid during the 2021-2022, 2022-2023, 2023-2024, and 2024-2025 award years. These institutions attended will be verified by using information from the National Student Loan Data System (NSLDS). Please complete the bottom portion of this form and request your official trancripts.

*If Moraine Park already has one or all of your official transcripts, please indicate that on the form below.

2. If you did not earn academic credit at each institution you attended during this time frame, please provide a letter outlining the reason(s) you failed to earn academic credit. Third-party documentation of this explanation is required. Academic credit is considered to be earned if the academic records show that the student completed any credit hours or clock hours. Please complete the bottom portion of this form and upload your letter.

Documentation will be reviewed by the Financial Aid Appeals Committee, and students will be notified of an official decision in writing. Providing all requested documentation does not guarantee student eligibility.

If you did not earn academic credit at each institution attended and if you are unable to provide sufficient explanation and documentation of the circumstances that led to the failure(s), you will be ineligible to receive further financial aid. Students with a UEH that are determined to be eligible for aid may be subject to additional requirements.
