As a student taking courses in the Foundations of Teacher Education Program, you are required to follow and abide by the requirements and expectations stated in this handbook.
It is requred that the student sign, date, and submit this form no later than the second week of class, each semester, while enrolled in a Practicum course.
Read the statements below and initial each box to identify that you have read and understood them.

I understand that if I do not follow and abide by these requirements and expectations that I may be removed from the Foundations of Teacher Education Program.

I am aware of my responsibillity to pay the annual DOJ name-based background check fee that will appear on my Lakeshore Student Account.
