Please read each of the following statements and select "Yes" in agreement you understand the statement.

1. I understand that all San Jacinto College formal communication is sent to my San Jacinto College email address.

|
2. I understand I am responsible for payment. Veterans Services department does not hold registration before or after Payment Deadline. If I submit a request for certification less than four weeks before the term and am unable to start a payment plan, I may request a deferment if using Chapter 33 and Chapter 31. Speak to a school certifying official after submitting your certification request or email Veterans@sjcd.edu with your deferment request.
**Note: Chapter 30, Chapter 35, 1606, and Tuition Assistance recipients are not eligible for deferments.**

3. Veterans and dependents can qualify for Financial Aid along with state/federal veteran benefits. I understand to receive financial aid I must submit a FAFSA application at FAFSA.GOV. After I have applied for FAFSA, my personal college financial plan is available to me in my student account.

4. I understand I should submit my request for certification four weeks before the semester begins to ensure application to my student account.

6. I understand school certifying officials review certifications requests “first submitted– first certified” order. Submitting a certification request three weeks before classes begin does not hold enrollment. Certifications take 7 – 10 business days to review/certify during non-peak periods.

7. I understand I must submit a veteran request for certification every semester to use Texas State Hazlewood, DOD tuition assistance, and/or Veterans Administration federal benefits.

8. Independent and distance learning “Online”: If using your post-9/11® GI Bill benefits while taking only distance-learning or “online” courses, I will receive a housing allowance based on 50% of the national average.

9. I understand that I must pay all past-due balances before the Business Office applies future term benefits. The Registrar’s office will drop future term courses for accounts without a zero balance.

10. I understand if my benefits are exhausted, I am responsible for the debt to the college and/or the Department of Veterans Affairs.

11. I understand that I am responsible for any debts owed to San Jacinto College not paid by my benefits.

12. I understand the drop/withdrawal calendar for refund processing is on the San Jacinto College website. Any drops or withdrawals are processed in accordance with the published timeline. San Jacinto College Refunds

13. Submission of a request for certification does not constitute a complete request for certification. A school certifying official will return an incomplete request for certification and the request will not be processed. If a request for certification is returned, you must resubmit all documents again.

14. I understand receipt and evaluation of all previous college or university, and military transcripts is required for certification of benefits.

15. I understand requests for future semesters requiring review of GPA, academic progress, course repeats, and/or approval of the Hazlewood appeal committee are not processed until the end of current term.
a. I am responsible to hold future registration if my benefits are pending appeal.
b. Contact your school certifying official if you qualify for a deferment.

16. Completion of pre-requisites does not necessarily equate with acceptance into a program. In addition, pre-requisite courses for a program must pull into your current degree for veteran benefits to cover tuition and count towards monthly housing allowance.

CHANGES IN ENROLLMENT OR BENEFITS
|
17. I understand it is my responsibility to review eligibility for benefits and promptly notify the college and the Veterans Administration (VA) of any changes, including withdrawal, or errors in my enrollment/certification.
a. Changes to registration and amendments to correct a certification with the VA may result in student debt to the college and/or the VA.

18. I understand if I change my enrollment after my initial certification, I may be responsible for a debt to the VA or San Jacinto College.

19. I understand that federal veteran educational benefits and state Hazlewood benefits cover courses applying to my current degree plan only. Any change to certified enrollment requires submission of a request for “schedule change” certification.

20. I understand that drop/withdrawal dates are on the SJC website. San Jacinto College Refunds If a hold prevents registration or dropping a class, I may contact Veterans Services or Veterans@sjcd.edu during college business hours to assist with the hold to drop the course in a timely manner.

21. I understand I must attend and participate in registered courses. Post 9/11® (Chapter 33) and other federal education recipients are required to verify enrollment monthly with the Department of Veterans Affairs. Verify Post 9/11® Enrollment

22. I understand if I am deployed to military service, I should notify San Jacinto College Veteran Services Veterans@sjcd.edu and my instructors immediately upon receipt of the orders.

23. I understand if I experience mitigating circumstances, I should notify San Jacinto College Veteran Services Veterans@sjcd.edu without delay to discuss withdrawal options, incomplete grades, and any possible debt with the college, DOD, or VA.
