This form is to be used by students who were suspended at the end of the SPRING 2025 semester. This form can be saved as a draft and returned to it at a later time.
If you were suspended at the close of the Spring 2025 term and wish to enroll/remain enrolled in classes at MSU for the FALL 2025 term, you may schedule an appointment to participate in the either the May or August Scholastic Appeal Sessions. These appeal meetings will take place on Zoom on the following dates:

- 12:30 p.m. - 4:30 p.m. (CST)
- Wednesday, May 28th
- 8:30 a.m. to 12:10 p.m. (CST)
August
- Monday, August 11th
- 12:30 p.m. - 4:30 p.m. (CST)
- Tuesday, August 12th
- 8:30 a.m. to 12:10 p.m. (CST)
You will be prompted to schedule an appeal meeting AFTER you have successfully submitted this suspension appeal form. Below outlines the steps to appeal:
1. Complete and submit this suspension appeal form by 12:00 p.m. (CST) on
- Monday, May 26th for the May appeals sessions
- Sunday, August 10th for the August appeals sessions
2. Schedule your appointment to meet via Zoom with the MCHHS Scholastic Appeals Committee by 5:00 p.m. (CST) on
- Monday, May 26th for the May appeals sessions
- Sunday, August 10th for the August appeals sessions
- The scheduling link will be provided to your Missouri State email address upon successful submission of this appeal request form.
3. Participate in your 20-minute appeal meeting on
- Tuesday, May 27th (12:30 - 4:30 p.m.)
- Wednesday, May 28th (8:30 - 12:10 p.m.)
- Monday, August 11th (12:30 - 4:30 p.m.)
- Tuesday, August 12th (8:30 - 12:10 p.m.)
- YOU may choose whichever available date/time works best for your schedule
Please note - a reliable internet connection, working camera, and working microphone are required for participation in your appeal meeting. If you miss the above-listed deadlines for submission, do not report to your meeting on time, or fail to attend your scheduled meeting, your suspension will be upheld.
A final status decision will be posted to your advising notes on your My Missouri State portal no later than the day following your appeal meeting.
