Athletic Training Early Assurance Application

The information requested in this section is utilized to meet Federal and State requirements. Your optional but important responses will not be used in the admission process. College and universities are asked by many, including the federal government, accrediting associations, college guides, newspapers, and our own college/university communities, to describe the racial/ethnic backgrounds of our students and employees.

To be read by the applicant prior to submission of recommenders' information. The Family Education Rights and Privacy Act of 1974 (FERPA) is a federal law protecting the confidentiality of student education records. I understand that (1) I have the right not to consent to the release of my education records; (2) I have the right to receive a copy of such records upon request; (3) and that this consent shall remain in effect until revoked by me, in writing, and delivered to Marshall University, but that any such revocation shall not affect disclosures previously made by Marshall University prior to the receipt of any such written revocation. In accordance with FERPA, I the undersigned, hereby authorize the person receiving a request to write a letter of recommendation in which he/she may reference educational records and information, such as grades, GPA, internships, school activities, disciplinary issues, and other related information for the purpose of applying to the Marshall University Athletic Training Early Assurance Program.

By submitting this application, I hereby certify all my answers are true and accurate to the best of my ability. I hereby certify I have read and understand the information as presented in this application and I agree if accepted, to abide by the rules of the Athletic Training Early Assurance Program. I understand the Athletic Training Early Assurance Program will be governed by an honor code that requires honesty, integrity, and non-discrimination in my interactions with others, and that a breach of this code could result in expulsion from the program.
