Please read each of the following statements and select "Yes" if you understand and agree with the statement.

1. I understand that my certification request must be submitted in full no later than 10 business days before the first Payment deadline of the semester to prevent my classes from being dropped.

2. I understand that certifications are completed in a “first in” basis. Submitting documents less than 10 days before payment deadline will not guarantee my benefits will hold my enrollment. Certifications take 7 – 10 business days to review and certify. Submission does not constitute a package is complete.

3. I understand Veterans Services will not prevent my courses from being dropped after Payment deadline. I am responsible for payment.

4. I understand that my enrollment will not be certified by San Jacinto College Veteran Services until all previous college, university, and military transcripts have been received and evaluated by the college.

5. My first semester at San Jacinto College may require I start a payment plan or speak to a Veteran Services Coordinator about a short-term loan to hold my classes until all transcripts have been received and evaluated.

6. I understand state Hazlewood and federal educational benefit certification must be requested every semester I want to use these benefits.

7. I understand that my request for certification will not be reviewed until all documents have been received by Veterans Services.

8. I understand upcoming semester benefits will be processed, after turning in all required documents, after grades post, if required for GPA, academic standing, repeats, or appeal.

9. I understand that I am responsible for notifying San Jacinto College Veterans Services of additions, deletions, or changes in my schedule including a complete withdrawal to prevent overpayment from the Department of Veterans Affairs.

10. I understand that federal veteran educational benefits and state Hazlewood benefits cover courses that apply to my current degree plan only.

11. I understand that I must pay all course repeat and book fees per my educational benefit. If unpaid, then the last enrolled courses will be dropped until a zero balance is achieved in my student account.

12. I understand if San Jacinto College Veteran Services or I am notified my benefits are exhausted, I am responsible for the debt to the college and/or the Department of Veterans Affairs.

13. I understand that I am responsible for any debts owed to San Jacinto College not paid by my benefits.

14. I understand that all formal communication from SJC regarding my VA benefits will be sent to my San Jac email address.

15. I understand that drop/withdrawal dates are published on the SJC website. If I have a hold preventing registration or drops, I must contact Veterans Services during normal business hours to lift the hold so that I may drop in a timely manner.

16. I understand that the drop/withdrawal calendar for refund processing is published on the San Jacinto College website and any drops/withdrawals will be processed in accordance with the published timeline.

17. I understand I must attend my courses and if required by my educational benefit, verify enrollment monthly with the Department of Veterans Affairs.

18. I understand if I have mitigating circumstances or have been recalled with orders to active duty I should notify San Jacinto College Veteran Services.
