In an effort to provide Northwest Missouri State University students with quality health care and health education, Wellness Services is conducting this survey. This data allows the Wellness Center to determine areas of improvement. You must be 18 years of age or older to participate in this survey. Your completion of the survey is voluntary.
The survey takes approximately 10 minutes to complete, and your responses are completely confidential.

Your participation will have no effect on your grades, the care you receive at the Wellness Center, or your relationship with Northwest faculty or staff. You may refuse to answer any questions or stop at any time.
There is a risk of experiencing discomfort when disclosing personal information. Data that could be shared or published would be overall responses or comments from the survey without names or identifying components.
