Campus Facilities Modification Request Form
Please submit a Campus Facilities Modification request by using the form below. This form was created by the Campus Facilities Committee comprising of the following members: Alexis Devendorf (SGA Rep), Alex Gill, Kristina Kopp, Colleen Kumiega, Melissa Laidman, Tim Seil (Chair), Don Suchan, and Jeremy Trumble.
This form is for any member of the campus community to request any type of facility modification. This modification can include capital and non-capital requests. When requesting a modification please be as detailed as possible, price quotes can be included if applicable. For modifications to be considered for the following fiscal year, they must be submitted by March 15.
Also as part of this form, space and office location requests can be made. When submitting a space and office request, please include current use of the space and proposed use of the space. Please allow at least 15 business days for the space request to be reviewed.
Definition of Capital Request: Any renovation or construction projects that create a capital asset with an expected life span of two or more years, and have a total expected value of at least $1,000 (e.g., Classroom renovation, student support spaces, office relocations/expansions, etc.).
