Guidelines:
(1) This form must be completed in full. Please do not use abbreviations.
(2) Provide as accurate a project timeline and duration as possible. In accordance with federal guidelines, projects must be reviewed each year of their duration. For projects greater than one year duration, the investigator must submit a written progress report and request for renewal to the VVC-IRB chair. This must occur no later than eleven (11) months after the previous approval date, to avoid interruption of the project.
(3) Submit completed applications using this online form only, along with any necessary supporting information (e.g., funding source information; articulation agreements with other institutions; etc). There will be an option to "Upload Documents" to enable you to do this online.
(4) Submission MUST include the Informed Consent document to be used in the study. Again, the Informed Consent and all other supporting information can be submitted with this application via the document upload process provided.
(5) Submission must also include a description of the data and/or files to be maintained, duration they will be kept, and means of disposal to be used.
