Voter ID Enrollment Verification Request

In accordance with Wisconsin Statues Chapter 55.02 (6m)(f) in the state of Wisconsin, a Moraine Park student ID card when presented with proof of current enrollment will suffice for voter identification.

This form is intended for the sole purpose of requesting a current enrollment verification letter in combination with Moraine Park Technical College's student ID card to be utilitzed for voting purposes.

Student must confirm current mailing address.
If current mailing address is different than address on file, student must complete Student Record Change form.
