General Consent Form

GENERAL CONSENT FORM

I hereby irrevocably grant Onondaga Community College (the "College") and its successors and assigns permission to use my name, pictures, or photographs of me, any other form of likeness of me created, statements or testimonials made by or about me, and my personal athletic record in any and all of the College's publications, website entries, announcements, and other marketing, promotional and advertising materials for purposes of publicizing the College and its programs, and for puposes of producing, selling and otherwise distributing College related products such as commemorative items or other memorabilia, all of the foregoingin any form or media, without payment or any other consideration. In connection therewith, I hereby irrevocably authorize the College to edit, alter, copy, exhibit, publish and/or distribute all such materials.

I understand and agree that all such materials are or will become the property of the College and will not be returned. 

In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein any such materials appear. Additionally, I waive any right to royalties or other compensation arising or related to the use of the materials identified above.

I hereby hold harmless and release and forever discharge the College, its successors and assigns, and its trustees, officers, employees and agents from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this consent.

This consent and such use also extend to any third-parties with whom the College may contract for permissible promotions. This consent and such use do no extend to any use that is derogatory to me personally. This consent shall be interpreted to meet the requirements of FERPA and the New York Civil Rights Law for the use and release of personal information about me.

I am 18 years of age or older and am competent to contract in my own name. I have read this consent before signing below and I fully understand the contents, meaning, and impact of this consent. 

A photocopy of this consent shall be as valid as the original.
