Bookstore Laptop Request Form
For students who need a computer at home for their coursework and have excess financial aid, a laptop can be purchased through the bookstore and the charge can be applied to your school bill.
Please complete the form below to indicate the laptop you would like to purchase. The Student Accounts office will review your billing to determine if you have sufficient funds to cover the charge for the selected laptop.
If your funds will cover the cost of the laptop a transfer of the requested amount will be processed to your Lazer Card and you will be able to purchase the laptop at the bookstore. Please note that while most coursework
will be able to be completed on these laptops, some may not be able to run some more complex software. If you have questions about whether specific software is needed, contact your instructors.

I understand that by completing this form and signing below, I am giving Onondaga Community College permission to use my Federal Title IV aid to pay for the purchase of a laptop computer as indicated above. I understand that I must attend classes past the ninth week of the term (60% of the course) to earn my Federal Title IV Aid (including student and parent loans). I further understand that if I do not meet credit hour requirements, or if I receive less financial aid than anticipated, I will be responsible to repay the amount of all the purchases made using my LazerCard to Onondaga Community College. Additionally I understand all funds are non-transferable and non-refundable.
Authorizing Federal Title IV Aid for Non-Institutional Expenses
Federal Title IV financial aid funds are restricted to payment of current period tuition, fees, room and board. Program regulations permit students to authorize use of Title IV financial aid funds for non-institutional expenses such as books, supplies or campus debit systems such as the LazerCard.
