Instructions:

Please complete the following forms of the New Hire Paperwork process. Be sure to bring your I-9 documents to Human Resources, Wickes Hall 373, as soon as possible after the completion of the forms. You may need to create a dynamic form log in HERE.

Federal law requires all employers to verify all new employees are eligible to work in the United States. You will be asked to provide original identifying documentation in person. Acceptable forms of identification are outlined on page 3 of the I-9 form or view HERE on the USISC website.

Please note that the Saginaw City Tax Form is not electronic and will need to be brought in during your I-9 document verification appointment or emailed to HR@SVSU.EDU. Our office is open Monday through Friday 8:00a-4:30p or by appointment.

Forms Include:
1. I-9 Form - 1st page only and page through to "submit"
2. W4 Tax Document - 1st page only and page through to "submit"
3. MI-W4 - 1st page only and page through to "submit"
4. Saginaw Income Tax Form - Email to HR@SVSU.EDU or bring it to your appointment. Only if the Zip Code is 48601 or 48602
5. Staff & Faculty FERPA Acknowledgement Form
6. EEO-1 Voluntary Self-Identification Form
7. Voluntary Self-Identification of Disability Form

Please note that we are unable to process your New Hire Paperwork until we have received your original identifying documentation in person. Visit the New Employee Guide for further information.
